STAR ELEMENTARY SCHOOL
Please bring the following to registration:
- State certified birth certificate (not hospital copy)
- Immunization Records or Waiver
- Proof of Residency
- Social Security Card
Children enrolling must be 5-years-old on or before December 1, 2010. Parents do not need to bring their children to registration. Parents may wish to bring their calendars as we will be scheduling a vision and hearing appointment at registration.
